| Short Description |
This form is used by project engineers who construct or modify a dam. Within 30 days following substantial completion of construction or modification of a dam, the project engineer shall submit to the DSS a declaration stating that the project was/was not constructed in accordance with the DSS approved plans and specifications and construction change orders. The declaration form may be used or altered, as appropriate, by the project engineer. | |
