The Site ID form is available as a
Word or
PDF form. The Word
document can be filled out using your computer. The PDF form can be
printed and filled out with a typewriter or by hand. Sign the form in
ink and mail it to Ecology using the address at the top of the form.
Once Ecology has a signature on file, future Site ID Forms and Annual
Reports can be filed electronically using
TurboWaste.Net.
