Off-Site Identification (OI) Form
Who needs to complete the OI Form?
If you had an active RCRA Site ID number any time
during the Reporting Year and were a MQG, LQG, or TSDR you must fill out
the OI Form.
What is the purpose of the OI Form?
The OI Form is used to collect the name, address, and RCRA Site ID Number of all dangerous waste
handlers who you interacted with when you send waste off-site or when you receive
waste from off-site during the Reporting Year.
What information do I need to complete the OI Form?
You will need the name, address, and RCRA Site ID Number of all generators, transporters, TSDRs, or Special Waste Handlers
that interacted with you.
- Generators must report all the off-site TSDRs who
treat/store/dispose/recycle their waste, and transporters who haul
their waste.
- TSDRs must report all the generators who send them waste, and
transporters who haul the waste.
- TSDRs ARE NOT required to report SQG in Washington (Household
Hazardous Waste or federal Conditionally Exempt Generators).
How do I find the information I need for the OI Form?
- The TSDRs RCRA Site ID Number, name and address is found on the Uniform
Hazardous Waste Manifests for Shipments. A TSDR is called a "designated
facility" on the Uniform Hazardous Waste Manifest.
- The Transporter RCRA Site ID Number and name are also on the Manifest.
- Search for Transporter, TSDR, and LQG information in the
US EPA's RCRA
Info database. The
Resource Conservation and Recovery Act Information (RCRAInfo) database is
a national inventory of hazardous waste handlers. All LQG, Transporters, TSDRs
of hazardous waste are required to provide information about their activities.
Use the Hazardous Waste Query to find identification and location data for
specific hazardous waste handlers, and to find a wide range of information on
TSDRs.
