Revise Previously Submitted Information

A revised Site ID form can be submitted anytime throughout the year to update site information such as name, address, phone numbers, and waste activities. You can also submit a revised Site ID form as part of your Annual Dangerous Waste Report.

To revise your Site ID information go online using TurboWaste.Net to electronically complete and submit a revised Site ID form or contact Tonya Wolfe at 360.407.6023 or email at twol461@ecy.wa.gov to have a pre-filled form sent to you.

Please note changes to site location and ownership information can not be submitted electronically over the Internet because a paper Site ID form (with signature) is required. Contact Tonya Wolfe at 360.407.6023 or email at twol461@ecy.wa.gov to have a pre-filled form sent to you.
