EPA

If EPA waste codes apply to your waste stream you will need to enter them in Section A-3 of the GM Form and/or section 3 of the WR Form. EPA waste codes begin with a P, U, F, K or D. The EPA waste codes are described in WAC 173-303-081, -082, and -090. They are listed in WAC 173-303-090, -9903, and -9904. If you need to use more than six waste codes please list the remaining codes in the Comments Section of the form.

Washington State Only

If you decide Washington State-only waste codes apply to your waste stream, enter them in Section A-4 of the GM form and/or Section 4 of the WR form. Washington state-only waste codes begin with a "W ".  The State only waste codes are described in WAC 173-303-082, -090, 100, 104, and 9904 are listed here. If you need more than two waste codes, please list the remaining codes in the Comments Section of the form.
