FormsThe UIC rule was revised and adopted on January 3, 2006.The registration form was revised to reflect changes in the rule. The following registration forms are available.
Note: The Environmental Protection Agency has UIC well classifications. Please refer to the EPA Class 5 Well Types page to specify the type of well(s) for the forms.
Ecology requires latitude and longitude information for all UIC wells. Latitude and longitude information allows Ecology to locate the well on a map. The Washington State Department of Health has a website that provides the latitude and longitude for a street address: http://ww4.doh.wa.gov/scripts/esrimap.dll?Name=geoview&Cmd=Map
For stormwater wells built after February 3, 2006, Ecology requires the ground water depth and the local geology of the area around the UIC well location. Visit Ecology’s Water Resources drinking water well log viewer at http://apps.ecy.wa.gov/welllog/. The website may have a well located within a ¼ mile from your UIC well and could provide useful information when completing the questions for new wells.
A Well Log is a Well Report that describes the location, ownership, construction details and lithology (subsurface material) of a completed well. The well log viewer web site enables you to search for wells that have well reports and to view the well report using a variety of search tools.
