Permit application forms for discharges to surface waters

These EPA forms are required to apply for a National Pollutant Discharge (NPDES) permit for discharge of wastewater to waters of the U.S. These forms are available in Adobe format from the EPA website or in Microsoft Word format as electronic fill-in forms. The forms in Word format are listed below. Note: These application forms require a determination by an Ecology permit writer of the parameters to be measured in the effluent as part of the application process. Contact the regional office permit coordinator before submitting an application.

EPA Form 1 – General Information – to be completed with the following EPA forms 2B, 2C, 2D, 2E, and 2F.
EPA Form 2A – Application for permit to discharge wastewater from Publicly Owned Treatment Works.
EPA Draft Instructions for completing Form 2A (Adobe format)
EPA Form 2B – Application for permit to discharge wastewater from concentrated animal feeding operations and aquatic animal production facilities.
EPA Form 2C – Application for permit to discharge wastewater from existing manufacturing, commercial, mining, and silvicultural operations.

EPA Form 2D - Application for permit to discharge process wastewater from new sources and new discharges.
EPA Form 2E – Application for facilities which do not discharge process wastewater.

State permit application forms for discharge to a POTW or to ground

These forms are required to apply for a State wastewater discharge permit for discharge to a POTW or to ground. The forms listed below are available in Microsoft Word (.doc files) and Adobe Acrobat (.pdf files) electronic form format. These application forms require that the regional permit writer determine the parameters to be measured in the effluent. Please contact the regional office permit coordinator before submitting these forms.
