SEPA FormsThe following forms can be found in either the SEPA Rules (Chapter 197-11 WAC) or in the SEPA Handbook (Ecology publication # 98-114). They are presented here in a variety of formats to aid in access and use of the forms. Individual lead agencies may make changes to forms, as described in WAC 197-11-906(4): "The forms in Part Eleven shall be used substantially as set forth. Minor changes are allowed to make the forms more useful to agencies, applicants, and the public, as long as the changes do not eliminate requested information or impose burdens on applicants. The questions in Part Two of the environmental checklist shall not be altered" (note: this refers to Part B of the checklist).
